We are open to new and innovative ideas to improve upon the care and delivery of services for our region.

The Carlton County Project

In July of 2014 Integrity Health Network was awarded a State Innovation Model E-Health Development Grant entitled the Carlton County Connects Project. This project will pull together the key health care providers of the county to identify how to move forward the health information exchange needed for the citizens. Through a collaborative effort of 15 organizations (including Carlton County Public Health and Human Services and Arrowhead Health Alliance), the Carlton County Project will assess the current infrastructure, identify gaps in information exchange, review solutions, and define a process for moving the planning into implementation.. Carlton County is rural area where health providers are often separated by significant distances and need better ways to assure communication and continuity of care. This project will create a plan that brings together hospitals, medical clinics, long-term care, public health, behavioral health, and social services in a way that improves health care outcomes and reduces costs due to redundancy and misinformation.
